Weapon duplication

I think everyone has been in those anarchy games where one of us pesky europeans or a no-gooder on wifi jumps in and a couple of weapons get duplicated. From what I understand this happens because of packet loss and is a side-effect of the way the network code works.
However I have been in a couple of games (out of hundreds, so really rare) where the duplication goes absolutely batshit insane crazy even though the packet loss is relatively mild. A recent example would be a dcl I played with slurpy and that we ended up calling off towards the end because it was getting a little out of hand.
At the time I even thought we hit object limit because my shots didn't really seemed to be registered on his end but upon closer inspection of the demo that might have just been some bad aim/leading from my end:) The hundreds of smarts were enough reason to call it off regardless I think.
Anyways, I was wondering if this duplication bug is maybe seperate from the normal incidental duping and if anyone else has seen it happen or if it is maybe even known what causes it.
